Please note that our shopping cart automatically assigns a $9 shipping charge to your payment. We have no way of over-riding the shipping fee at this time. YOUR ACCOUNT WILL NOT BE CHARGED THIS FEE. This form is used to collect your payment information. All transactions are processed manually.
